Health Checks and Immunisation

Dear all,

The School Health Services will be in our school in February and there will be 2 health checks for your child. By now, you would have received 2 forms and a letter.

Please complete item 1 and 2 and return them with your child's Health Booklet to me by 7th Feb, Wednesday.

1. P1 Immunisation Consent Form (For completion by parents, returned to Form Teacher and inserted into health booklet)

2. Medical Information Form (For completion by parents, kept by form Teacher till heath team arrives)

3. Letter Re Sch Health Service Visit (Blue in colour and for parents' retention)

For children who do not have the health booklets, you may purchase one at $1.50 each. Kindly attach a short note and the money to me so that I can help your child get the health booklet.

The dates of the health checks are as follows:

13th February, Tuesday [Audiometry]
14th February, Wednesday [Medical Examination+ Immunisation].

Please ensure your child is in PE attire (shouldn't be a problem since there is PE on both days).
